SCDL 2012 Rewards Program

Southern Cross Dental Laboratories (SCDL) is proud of the quality and range of dental technical services it provides to its customers and appreciates their on-going loyalty. In return we aim to offer you the opportunity to participate in a selection of the most exclusive and premier events, delivering to you professional development across a range of areas of clinical interest as well as the opportunity to interact with your professional colleagues in a relaxing environment. To do this, we have established a rewards program that will allocate points towards benefits based on the amount of laboratory services you utilise.

Terms and Conditions:

Participation in the SCDL Rewards program is free. Registration for the program will be automatically activated when dentists who have signed up with SCDL initially log into their Virtual Practice Site (VPS). If a dentist does not wish to participate in the Program they will need to advise us at this stage.

Participation in the SCDL Rewards Program constitutes acceptance of the following terms and conditions:

1. THE SCDL REWARDS PROGRAM

1.1 The Program is operated by SCDL.

1.2 Participation in the program is open to:

  1. Any dentist who has signed up with SCDL as an individual in his/her own right.
  2. Any dentist working in a partnership where the partnership has signed up with SCDL.
  3. Any employee dentist where the principal dentist has signed up with SCDL.
  4. Any dentist working for a Corporation where the Corporation has signed up with SCDL.

1.3 A dentist or dental group who signs up with SCDL is allocated a unique account number.

  1. For an individual dentist, this number will be used to identify them as a participant in the Program.
  2. For partnership, employer-employee and Corporate accounts it will be necessary for the account holder to either:
    1. Authorise for account numbers to be created for each individual dentist (as a sub-group under the principal account holder) or
    2. (a) Maintain their own records and allocate points within their own business.
      (b) Advise SCDL of the points distribution at the end of the allocated period.

1.4 SCDL reserves the right to make changes to these terms and conditions from time to time and without notice. The most up to date copy of the terms and conditions will appear on the SCDL website (www.scdlab.com).

1.5 SCDL reserves the right to cancel or suspend the Program at any time and without notice.

1.6 Eligibility to participate in the Program may be terminated or suspended by SCDL without notice if:

  1. You do not comply with these terms and conditions.
  2. You are more than three months in arrears in paying your SCDL account.
  3. You cease to have an active account with SCDL; or
  4. SCDL ceases to operate the Program for any reason.

 

2. THE REWARDS

2.1 Rewards available under the Program are advised in the Rewards Program booklet distributed annually, on the SCDL website and in other promotional material issued by us from time to time. All information is accurate at the time of publication but may change without notice.

2.2 Rewards are subject to availability and any other specific terms and conditions which may be imposed. Unless otherwise stated no Reward may be used in conjunction with any other offer.

2.3 Rewards are allocated to individual account numbers only and cannot be sold, transferred, assigned or otherwise dealt with except as outlined at section 1.3 of this document and with the consent of SCDL.

2.4 Reward points will be allocated to your account based on your account number only. If an incorrect account number is recorded by you on your documentation, the points will be allocated to that account.

2.5 The Rewards Program will offer a range of entitlements at four status levels – Bronze, Silver, Gold and Platinum.

2.6 Rewards at each status level are offered as stated and no compensation will be payable if a dentist fails to take up or use all of the entitlements for that status level in a given financial year (see 2.8).

2.7 There will not be any entitlement to compensation if SCDL elects to terminate or suspend the Program.

2.8 Reward points will accumulate over a financial year (1 July to 30 June). At the end of that year all reward points will be deleted and new accounts commenced for the following year.

2.9 One (1) SCDL Reward point will be allocated per dollar spent unless advised otherwise as part of a specific program, promotion or contract.

2.10 In July of each year SCDL will forward to each participant a Rewards Program Redemption Choice Pack or equivalent from which rewards may be chosen depending on your status level.

2.11 SCDL takes all reasonable care to ensure that information published in relation to the Program is accurate. Any corrections will be advised via our website.

2.12 Offers cannot be combined with any other offer or promotion.

2.13 Unused reward points or dollars cannot be carried over to the following year.

2.14 Reward dollars cannot be redeemed as cash.

2.15 In the event of any conflict or dispute, SCDL has the absolute sole discretion to determine if a claim is valid.

2.16 SCDL will not be responsible for or liable for any injury or damage, accident, loss, delay or irregularity that may be caused to persons or property in connection with any goods or services purchased as part of the Rewards Program.

2.17 Liability for any taxation assessment on rewards in this program is the sole responsibility of the recipient. It is strongly recommended that the recipient(s) of rewards consult with a tax agent or accountant regarding any taxation liability as individual circumstances may differ.

2.18 SCDL will compute each participant's points earnings and report them on the customer's VPS site, except for Corporate dentists as per section 1.3(b) (ii) (b)whose points allocation will be advised to SCDL.

2.19 Reward points are an indication of a customer's balance at any given time.

 

3. PRIVACY

3.1 Information on how we handle your personal information is explained in our Privacy Policy Statement available on the SCDL website.

 

4. DEFINITIONS

In these terms and conditions: "SCDL", "we" or "us" means Southern Cross Dental Laboratories.

"You" means a person who is registered dentist and who has an account with SCDL, or a dental group or Corporation which is eligible to participate in the Program as per section 1.3 "Program" means the SCDL Rewards Program to which these terms and conditions apply.

 

5. UNSUBSCRIBING

5.1 If at any time you decide to no longer participate in our SCDL Rewards Program you can telephone us on (02) 9362 1177 or email us on australia@scdlab.com and advise us of your decision. Your name and any existing Rewards points will be removed from our SCDL Rewards Program register.

5.2 Non participation in the SCDL Rewards program will not in any way affect the provision of laboratory services by SCDL.

2.20 All reward points are subject to audit.

2.21 Customers can only earn a maximum of $10,000 in awards (per financial year) and the total reward dollars redeemed by an individual customer cannot exceed their current reward dollars available.

2.22 Each reward can only be redeemed once (per financial year).
